Relationship Between Telomere Length and Pulmonary Toxicity in Patients with Locally Advanced Unresectable Non-small cell lung cancer (NSCLC)

Introduction: Telomere length (TL) is involved in tissue repair. Pulmonary toxicity (PT) due to radiotherapy (RT) is a complication of NSCLC treatment, being a risk factor under study. Objective: Analyze the relationship between TL and PT due to RT in patients with locally advanced unresectable NSCLC treated with concurrent chemoradiotherapy and immunotherapy (IT) in PDL-1 positive patients. Methodology: Unicenter prospective study of 90 patients enrolled between 2018-2021. Clinical, analytical, tumor, treatment data and TL were collected. TL was obtained from buccal samples classifying patients in percentiles according to z-score. Patients were divided into two groups according to the severity of PT: G1-G2(mild)/G3-G5(severe). Results: Ninety (90%) were men with a mean age of 72 years(SD 6.3). Predominant histology was squamous (54%) and 40% received IT. 81% developed G1-2 toxicity and 19% G3-G5 with significant differences only in dosimetric variables; V20: 20Gy vs. 26Gy (p=0.002) and V5: 46Gy vs. 54Gy (p=0.006). No significant difference was observed between PT and TL. In the subgroup analysis by IT, lower PT was observed in patients with TL above percentile 25(Fig. 1) erj;66/suppl_69/PA1802/F1 F1 F1 Conclusions: No relation was found between LT and PT. However, patients with TL above 25th percentile treated with IT had lower risk of developing sever PT. Further studies are needed to validate this.
