Telomere Length and Survival in Patients with Locally Advanced UnresectableNon-small Cell Lung Cancer (NSCLC): Is There a Relationship?

Introduction: Telomerase is overexpressed in 80% of tumors, preventing telomere shortening and providing cellular immortality. Immunotherapy (IT) after concurrent chemoradiotherapy (CCRT) has a benefit in overall survival (OS) in patients with NSCLC PDL-1 positive. Objective: to analyze the relationship between telomere length (TL) and OS in patients with NSCLC PD-L1 positive treated with CCRT and IT. Methodology: Unicenter prospective study between 2018-2021. Clinical, analytical data, tumor and treatments characteristics were collected. TL was obtained by buccal swab and patients were classified into two groups based on z-score percentile: percentile <25 and >25. OS was analyzed during follow up until death/last visit using the Kaplan-Meier method. Results: Ninety patients were included; 90% were men, mean age of 72 years(SD 6.3), 54% were squamous cells and 40% received IT. 83% of patients showed lower TL than healthy population. No differences in OS were observed between the two study groups(p>0.9). In subgroup analyisis, patients who received IT treatment with TL percentile>25 had better OS than dose percentile<25(p=0.046). No differences on OS were observed in patients without IT(Fig. 1). erj;66/suppl_69/PA1803/F1 F1 F1 Conclusions: NSCLC patients have lower LT than healthy ones. Patients treated with IT and LT above the 25th percentile showed better OS. Further studies to validate this are required.
