Minimally invasive PVR surgery-MIPS

Background: Proliferative vitreoretinopathy (PVR) is one of the main reasons for recurrent retinal detachments, usually requiring multiple re-surgeries. This technique of minimally invasive PVR surgery (MIPS) provides a simpler and cost-effective way of treating PVR in selected cases. Purpose: To demonstrate the surgical maneuvers and outcomes of MIPS Synopsis: Recurrent retinal detachments with PVR pose significant challenges for vitreoretinal (VR) surgeons, often necessitating lengthy, complex, and multiple surgical interventions, thereby increasing associated costs. [1] This series presents a few cases where PVR led to tractional recurrent retinal detachment in eyes filled with silicone oil. In these instances, only two trocar entries were made—one for the light source and one for forceps. Without removing the existing silicone oil, multiple PVR membranes were peeled under the oil. Holding and peeling the membranes under silicone oil is not very difficult. [2] The silicone oil facilitated membrane edge identification and peeling by providing contrast at the edges and counter-traction during the peeling procedure. Silicone oil top-up was done if needed. The retinas were successfully reattached, thereby conserving valuable surgical time, effort, and cost. The described technique may be used as a standalone procedure in selected cases or as an initial step in managing complicated PVR cases using conventional surgical methods. Highlights: Simple and reproducible technique. This technique can be the sole procedure in selected cases and can serve as the first step of a complicated PVR case managed by conventional surgical options.
