The pro-cancer and immunosuppressive activity of macrophage-transformed cancer-associated fibroblasts in oral squamous cell carcinoma

INTRODUCTION: Cancer-associated fibroblasts (CAFs), as a crucial component of the tumor microenvironment (TME), exhibit heterogeneity, and macrophage-myofibroblast transition (MMT) has been demonstrated to be one of the origins of CAFs. However, the existence and functional features of MMT cells (MMTs) relative to general CAFs in tumors, especially in oral squamous cell carcinoma (OSCC), have not been fully revealed. OBJECTIVES: This study is intended to understand the origin and functional features of MMT and the underlying mechanisms of its formation and functionality. METHODS: The presence of MMT in OSCC was determined through single-cell RNA sequencing (scRNA-seq) analysis, immunofluorescence of OSCC pathological sections, and flow cytometry of OSCC allograft tumors. The origin and potential formation mechanism of MMT were investigated through scRNA-seq pseudotime analysis and in vitro induction of BMDM. Subcutaneous OSCC tumor model mice were employed, in combination with macrophage depletion, adoptive transfer of BMDM-derived MMTs (BMDM-MMTs), and flow cytometry and immunofluorescence staining were carried out to investigate the role of MMT in OSCC. Further in vivo and in vitro experiments, in conjunction with scRNA-seq, were performed to explore the possible mechanism through which MMT exerts its function in OSCC. RESULTS: MMT was confirmed to exist in OSCC. The addition of tumor-conditioned medium and exogenous TGFβ1 promoted MMT. Meanwhile, it was found that the adoptive transfer of BMDM-MMT significantly enhanced tumor progression and fibrosis in OSCC tumor-bearing mice. Single-cell sequencing and in vitro and in vivo experiments jointly verified that MMTs promoted MDSCs homing and functionality via CXCL10 to promote the immunosuppressive microenvironment of the tumor. CONCLUSION: These results imply that macrophages can transform into CAFs via the activated smad3 pathway by TGFβ1 derived from tumor cells in OSCC, and MMT exerts protumoral and immunosuppressive functions in OSCC through the CXCL10 pathway. MMT may become a novel therapeutic target for the immunotherapy of OSCC.
