Abstract This chapter discusses preconception counseling and pregnancy in epilepsy. Reproductive-aged women on antiseizure medications (ASMs) might experience challenges related to medication noncompliance, unknown drug–drug interactions, and reproduction. Therefore, it is important for clinicians to routinely review patients’ reproductive plans, contraceptive options, and medication risks with the intent of modifying, supplementing, or discontinuing medical therapy when appropriate. Teratogenicity of various ASMs is reviewed. Physicians should regularly re-examine the appropriateness of using potentially harmful therapies in their patient population. The U.S. Food and Drug Administration revised the “Pregnancy and Lactation Labeling Rule” to assist clinicians in evaluating drug risks to pregnant or nursing mothers.
