Normally, nearly all hemoglobin is contained within circulating red blood cells.Circulating free hemoglobin is not detectable in significant quantities in adults.Several pathways have been identified for the rapid scavenging and removal of free hemoglobin from circulation, including the plasma proteins haptoglobin and hemopexin.1,2 In some pathophysiological conditions, the presence of free hemoglobin contributes to endothelial injury through mechanisms such as nitric oxide scavenging, oxidative stress, endothelial activation, and dysfunction of the endothelial barrier.3
