Background: When chronic obstructive pulmonary disease (COPD) is newly diagnosed in the mild to moderate stages (Global Initiative for chronic obstructive Lung Disease [GOLD] stage 0-2), it is difficult to identify patients at increased risk of death. Methods: Therefore, we examined the prediction of mortality by the biomarkers Copeptin (COPAVP), midregional adrenomedullin (MRproADM), pro atrial naturetic peptide (MRproANP), and Fibrinogen in subjects with stable COPD GOLD 0-4 and GOLD 0-2 (diagnosis COPD for less than 5 years) of the COSYCONET cohort (COPD and Systemic Consequences - Comorbidities Network). Results: 235 subjects of 2253 subjects with stable COPD 0-4 and 43 subjects of 655 with GOLD 0-2 (diagnosed less than 5 years ago) died during the 6-year follow-up period. In both groups, Cox regression after adjusting for confounding factors (COPD characteristics, other risk factors, cardiovascular disease, log10troponine, and creatinine) revealed that the fourth quartile of MRproANP is a robust biomarker of all-cause mortality. Conclusion: In contrast to the biomarkers COPAVP, MRproADM and Fibrinogen, the fourth quartile of MRproANP is a robust biomarker of all-cause mortality even in subjects with early stages of COPD and a recently diagnosed COPD.
