The impact of antiphospholipid antibody syndrome and/or systemic lupus erythematosus on first trimester combined screening for preterm preeclampsia – data from the IPSISS cohort

Objectives: The FMF preeclampsia screening combines maternal and biochemical factors.Maternal co-morbidities such as lupus erythematosus(SLE) and anti-phospholipid syndrome(APS) are part of this model. Our aim was to examine the prevalence and impact of APS/SLE in our cohort. Materials and methods: Patients in the IPSISS database were divided into group(1)with APS/SLE, and group(2) those without. All women were screened according to the FMF London algorithm and those at risk for pPE were treated with LDA 150mg/d. For better access to medical information we restricted the analysis to women from University Hospital Bern. Results: 1976women were screened for pPE.Immunologic co-morbidities were recorded for 46(2.33%) of them;38(1.92%) with APS, and 11with SLE(+/- APS) (0.65%). The median(range) calculated risk for pPE was significantly higher in group1(1:103 [51-672]) than in group2:1:694 [229-1997]; p<0.0001).All but one(aspirin allergy) women of group1 were treated with LDA,whereas only 215/1927(11.2%) of group2 received LDA(p<0.0001). Apart from a significant difference in MAP(higher MoM in group1),there was no difference in the biochemical screening parameters.Although in group1 the incidence of pPE and SGA was lower,these differences were statistically not significant(group1: 1/35 [2.86%] vs. group2: 28/833[3.36%]; p=ns)(group1: 3/32[9.38%] vs.group2: 122/855[14.27%],p=ns). After reviewing the medical records,only19(50%) of the“APS” patients fulfilled the Sapporo criteria while 18(47.4%)were diagnosed with “non-criteria APS”. Conclusion: Women with immunologic co-morbidities had a higher chance to receive LDA.The incidence of placental related complications(pPE,SGA) was lower in women with APS/SLE. The diagnosis of APS was set widely and even women who did not fulfill all Sapporo criteria were considered as APS positive. Publication History Article published online: 01 October 2024 © 2024.
