The chronic form of coronary heart disease (CHD) is one of the most common types of cardiovascular pathology in the Russian Federation. The main causes of myocardial ischemia are a decrease in coronary reserve, as well as a decrease in coronary blood flow due to atherosclerotic stenosis. When cardiac muscle perfusion is impaired, energy deficiency occurs in cardiomyocytes and an ischemic cascade develops, leading to oxidative stress. In conditions of chronic ischemia in ischemic heart disease, therapy should be aimed at maintaining the energy-synthesizing function of mitochondria by activating the least energy- and oxygen-consuming pathways of ATP synthesis, combating oxidative stress and preventing the death of cardiomyocytes. Cytoprotector drugs meet these requirements. One of their most famous representatives is Mexidol — original (biological reference medicine) ethylmethylhydroxypyridine succinate. Anti-ischemic action is achieved by combining of antioxidant, antihypoxic and membrane-stabilizing pharmacological effects. The review presents the experience of using ethylmethylhydroxypyridine succinate in patients with chronic ischemic heart disease from the perspective of studying their effectiveness and safety using clinical, instrumental and laboratory research methods.
