Exploring modes in psychosis

We present here findings regarding the mode experiences of our group with psychotic diagnoses, illustrating their subjective descriptions and highlighting patterns. The modes of punitiveness, vulnerability and extreme anger loomed large in the accounts of our participants, and all three of these were linked with aspects of psychosis. Simplified names are used here for these modes, whose sources are usually traced to childhood experiences, although diverse causes are possible. Given the proposed origins of modes, this picture suggests potential histories of trauma and unmet needs in early life, which indeed some of our participants described spontaneously. It is known that people experiencing psychosis very often carry significant histories of trauma, particularly of an interpersonal nature. Our participants’ accounts may illustrate some connections between earlier adversities, ongoing patterns of responding and relating, and aspects of psychotic experiences. Two dysfunctional coping mode types were coded in most participants’ accounts: compliant surrender and detached protector. Individuals connected activation of these coping modes with overwhelming emotion linked to other modes and symptoms but more frequently described using these to cope with repeated and distressing experiences in the mental healthcare system and in a stigmatising society.
