Dear Editor We thank S. Jones and co-authors for their interest in our article ‘Watch and wait after neoadjuvant treatment in rectal cancer: comparison of outcomes in patients with and without a complete response at first reassessment in the International Watch & Wait Database (IWWD)’1. Jones et al. raise some important questions concerning our work and with this response we will try to clarify. Out of 249 patients with T1 or T2 disease, 119 (11.8 per cent of all included patients) had N0 disease. While we lack individual information on the reasons for treatment, neoadjuvant treatment can be considered for select cases with N0 disease, such as those with very distal tumours or those enrolled in clinical trials. We acknowledge that the extended recruitment period may introduce stage migration bias. To mitigate this, we specifically included patients who underwent both MRI and endoscopy at reassessment. These modalities are still widely used and known to provide the most accurate diagnostic information in current practice. Unfortunately, due to the construct of the IWWD, immortal time bias is unavoidable. Patients are retrospectively included in the registry upon achieving a cCR, which by default can only occur after the first reassessment. While we acknowledge that organ preservation and detection of metastasis can be influenced by the frequency and quality of follow-up, we believe that this factor did not significantly impact our results. The participating centres in the IWWD are experienced W&W centres with their own protocols, adhering to strict follow-up guidelines and harbouring considerable expertise in managing rectal cancer patients. Our study does not encompass all near complete responders at the first reassessment, limiting conclusions about outcomes at this specific point. However, we can emphasize that if a complete response is eventually achieved, the patients’ chances of favourable outcomes remain comparable.
