Background: Chronic lung allograft dysfunction (CLAD) is the major long-term complication after lung transplantation, clinically manifesting as 2 main phenotypes: bronchiolitis obliterans syndrome (BOS) and restrictive lung allograft syndrome (RAS). Janus kinases (JAKs) are intracellular signaling molecules, and play a role in regulating development, proliferation, and activation of several immune cell types involved in CLAD pathogenesis, including dendritic cells, macrophages, T cells, B cells, and neutrophils. The aim of this study was to evaluate whether the JAK-STAT pathway was upregulated in CLAD. Methods: Explanted CLAD lungs (both BOS and RAS, n= 11 and 14) and unused donor lungs as controls (n= 11) were frozen and processed into lung tissue cores (n= 43, 54 and 43 for BOS, RAS and control). RNA extraction and tissue RNA-sequencing was performed on these cores, and gene set variation analysis (GSVA, a gene set enrichment analysis that estimates variation of pathway activity) of the JAK/STAT KEGG gene list (Kyoto Encyclopedia of Genes and Genomes) was carried out. Results: Genes included in the JAK/STAT pathway (e.g. AKT1, BCL2L1, JAK1, JAK2 …) showed significantly increased expression in both BOS and RAS, compared to controls (Figure 1). Conclusion: JAK/STAT signalling might play a key role in the chronic inflammation and fibrosis process in CLAD, which suggests that JAK-inhibitors could have therapeutic potential in treatment (and possibly prevention) of CLAD.
