Delivering therapeutics to brain always remains an open challenge for drug delivery. Endothelial tight junction, astrocytes, and basal lamina of brain, which is collectively called as blood–brain barrier, serve as a tough nutshell for the restricted entry of solutes, metabolites, and molecular drugs. However, the new drug-delivery method and evolving techniques are aspiring to improve the therapeutic window and efficacy to overcome the hurdles of BBB and combat series of neurological disorders and diseases. The evolving method includes the application of nanotechnologies using active- and passive-targeting systems to enhance the neuronal bioavailability and therapeutic payload to the pathologically affected region of the brain. This approach can be adopted to treat various neurovegetative disorders as well as various types of brain cancer and gliomas. This chapter focuses on various rationale-based targeting strategies to deliver medicine to the affected brain and different modes of therapy that can be coupled with targeting methods to effectively transport biologics to the affected brain lesion.
