OC04.02: Fetal growth rate and cerebral blood flow redistribution in suspected late fetal growth restriction

To assess the association of fetal growth and fetal cerebral blood flow redistribution, umbilico-cerebral ratio (UCR) or short-term neonatal outcome in suspected late fetal growth restriction (FGR). This is a secondary analysis from multicentre observational study (TRUFFLE2) from April 2017 to July 2018. Women at 32-36 weeks with suspect of FGR (estimated fetal weight (EFW) p40) were monitored by 2-weekly biometry and Doppler assessment. UCR ≥0.9 was classified as abnormal and categorised for gestational age (GA) (32-33, 34-36 or ≥37 weeks) at first abnormal value or never abnormal. Fetal growth was calculated by the difference between birth weight and EFW at 3, 2 or 1 completed week before delivery, divided by the interval in weeks. 856 women were included after exclusion of 17 infants with major congenital anomalies. Fetal growth was lowest in the category with first abnormal UCR at 32-33 weeks (figure 1). In these, fetal growth was not significantly different from zero at one week before delivery. There was no difference in fetal growth between those with short term adverse or normal outcome in any of the GA at first abnormal UCR and interval to delivery categories. In women with suspected late FGR, fetal growth rate decreases during the last weeks before delivery, and this is most conspicuous in those with early abnormal UCR, whose growth is not significantly different from zero.
