VP01.07: The pregnancy outcomes of tubal and ovarian heterotopic pregnancies

Studies on heterotopic pregnancies have reported widely varying rates of miscarriage and live-birth rates of normally sited (eutopic) pregnancies. The aim of our study was to examine the risk of miscarriage of the live eutopic pregnancy following the treatment of concomitant ectopic pregnancy. This was a retrospective observational case-control study of tubal and ovarian heterotopic pregnancies carried out in three London Early Pregnancy Units between April 2000 and November 2019. We included those with live or potentially live eutopic pregnancies at the time of diagnosis and compared them to three singleton control cases each, matched for maternal age, mode of conception and gestational age at presentation. The primary outcome was the rate of miscarriage at 12 weeks of gestation and secondary outcome was live-birth rate. Sub-analysis was undertaken for pregnancy outcome according to treatment. A total of 52 heterotopic pregnancies and 156 matched control cases were included for analysis. 56% of heterotopic pregnancies were conceived by artificial reproductive therapy. 49 heterotopic pregnancies (94%) were located in the fallopian tube and 3 (6%) in the ovary. 49 (94%) were treated by laparoscopic surgery and three were managed expectantly. There was no difference in the rate of miscarriage in the heterotopic pregnancies compared to control cases (15% vs 17% respectively, OR 0.87; 95% CI 0.37-2.05). There was no difference in the live-birth rate between the two groups (95% heterotopics vs 91% controls, OR 1.83; 95% 0.39 – 8.63). Of those treated by laparoscopy, 84% were live by 12 weeks of gestation and 93% of those had a live-birth. Our study has shown that women who had undergone surgical treatment of heterotopic pregnancy with a concomitant live eutopic pregnancy had similar miscarriage and live birth rates as controls.
