A 63-year-old man with a history of orthotopic liver transplantation was presented to the emergency department with progressive right flank pain and nausea. On physical examination, we found a hernia cicatricalis that was painful on palpation. An abdominal CT scan showed acute perforated appendicitis, located in a hernia cicatricalis.
