Anterior cruciate ligament reconstrucion is one of the most performed procedures in today’s Orthopedic elective surgeries. Despite the high number there is a relative low infection rate reported between 0.001 to 1.1%. Early diagnostic is especially important as the main goal is to suppress the infection, but also to be able to maintain the graft and to prevent joint destruction. The case presents a deep, early joint infection following an ACLR with Staphylococcus aureus treated with early joint lavage, debridement and specific antibiotic treatment. Although all the biological and local parameters were normalizing the patient still presented fever and an exceedingly rare complication occurred due to Vancomycin administration. The patient had developed Vancomycin- induced fever.
