Background: We hypothesized that using speckle-tracking echocardiography and analyzing regional left ventricular (LV) mechanics, the early LV dysfunction can be found in chronic aortic regurgitation (AR). Aim of the study was to find differences in global and segmental (septal and free LV wall) strains and rotation in AR patients. Methods: We investigated 56 patients with moderate AR and 51 with severe AR. The control group consisted of 56 healthy subjects. All patients underwent standard transthoracic echocardiographic study and LV strain analysis was performed off-line using commercially available 2D strain software. The strain and rotation of the LV septal part were calculated as a strain or rotation average of anterior and posterior septum and inferior LV wall. The strain and rotation of LV free wall were calculated as average of strain or rotation of LV anterior, lateral and posterior walls. Results: Age, gender and body surface area did not differ between groups. In AR patients LV dimensions were higher than in controls (p , 0.05). LV systolic function was significant lower in severe AR group compared to controls, but it still exceeded 55%. Global longitudinal strain (LS) was reduced in both patients groups (p , 0.05) compared to controls. LS in septal LV part was attenuated earlier – in moderate AR (-18.4+2.79% vs -20.54+2.4%, p , 0.01), while longitudinal mechanics in free LV wall was impaired only in severe AR (-17.5+3.32% vs -20.1+2.49%, p , 0.05). Global circumferential strain (CS) and radial strain (RS) analysis did not show significant differences between groups. Septal LV CS was reduced in moderate and severe AR compared to controls (-23.0+3.49% and 21.6+3.67% vs -24.9+4.35%, p , 0.05). We found reduced LV basal rotation only in septal LV part in severe AR (-6.60+2.488 vs -8.29+3.028, p=0.02). [...].
