We thank Ballo et al. for their comments regarding the possible use of three-dimensional (3D) echocardiography for the optimization of cardiac resynchronization therapy (CRT). Three-dimensional echocardiography has been used to assess ventricular dyssynchrony1 as well as ventricular volumes and function.2 We agree with Ballo et al. that, to date, none of these measures have a defined role in CRT optimization. Current evidence necessitates optimization of the atrioventricular delay by optimizing the diastolic filling time using the iterative method. Pilot data have shown 3D echo to be of some use in the assessment of diastolic flow in patients with preserved ejection fraction.3 How this would perform in CRT candidates with depressed ejection fraction and often dyssynchronous function is unknown. It is also suggested that the estimation of stroke volume by 3D echocardiography may be of use for optimization. Importantly, no large study has shown the optimization of either the atrioventricular or interventricular delay by stroke volume estimation to confer any long-term clinical benefit.4,5
