The field of stem cell transplantation continues to develop rapidly, particularly in the use of alternative donors. Cord transplantation continues to evolve and is now established, though not yet optimized in adult practice. There is new interest in the use of haploidentical donor transplantation, resulting in almost all patients who need a transplant having a potential donor. Further extending the age limit upwards for transplants from family or unrelated donors continues with a better definition of patient suitability for transplant through comorbidity assessments. The use of reduced intensity conditioning regimens is now routine and can be applied to any stem cell source. While allogeneic transplants increase, the use of autologous transplants seems set to reduce further, as studies in acute leukaemia suggest it has no or only a limited role. As conventional dose therapy improves for lymphomas and myeloma, these areas may also see a reduction in the indications for autografting or a fall in the numbers of patients who may benefit.
