A Case of Sciatica due to a Sciatic Hernia

症例は68歳，女性．1カ月前からの右下腹部痛・右下肢痛が増強し，歩行障害を認めたため近医を受診．腹部CTにて骨盤底ヘルニアが疑われ，当科を紹介受診となった．腹部CTで右坐骨孔から脱出する小腸を認め，大坐骨孔ヘルニアと診断した．腸閉塞の合併は認めなかったが，坐骨神経痛を伴っていたことから，嵌頓の可能性を考えて緊急手術を行った．開腹すると右坐骨ヘルニアを認めたが，既に小腸嵌入は解除されていた． 腸管壊死所見はなく，小腸切除は行わなかった．ヘルニア門を単純縫合にて閉鎖した．術翌日，右下肢痛は消失しており，坐骨神経障害は改善していた．坐骨ヘルニアは極めて稀なヘルニアであり，小腸嵌入を伴う症例は14例しか報告されておらず，その中でも坐骨神経痛を伴う症例は4例のみである．今回の症例に若干の文献的考察を加えて報告する．
