カフェイン過量摂取の報告は米国では年間約4,600名であるが，本邦での報告は比較的少ない。カフェイン過剰摂取は生命を脅かす循環不全や神経学的異常を引き越こすことが知られている。本稿では，早期に血液透析を行い救命しえたカフェイン中毒の症例を経験したので報告する。症例は基礎疾患のない26歳の男性。研究用試薬カフェイン25gを自殺目的で摂取後，頻呼吸，嘔吐が頻回に認められ，内服1時間後に当院へ搬送された。来院時患者は興奮状態であり，頻脈，頻呼吸，振戦，筋緊張の亢進を認めた。心拍数は200回/分で多源性心室頻拍を認めた。心室頻拍はリドカイン投与に反応を示さなかった。カフェインの成人致死量の約10gをこえる25gを摂取しており，循環動態の破綻や難治性不整脈の出現が危惧されたため，内服後2時間，来院後1時間で血液透析を施行したところ，速やかに臨床所見は改善し，カフェイン血中濃度は，著明に低下した。不整脈や呼吸障害を伴う重症カフェイン中毒の症例においては，早期からの血液透析導入により良好な予後が期待できる。
