症例は 58 歳女性．1999 年 4 月上旬より咳，痰，労作時の息切れが出現した．胸部レントゲンにて異常影を認め，LDH, NSE 高値と肺生検所見から肺小細胞癌 (SCLC) と診断された．化学療法を施行し寛解していたが，2000 年 2 月に白血球の著明な上昇を認め，急性骨髄性白血病 (AML, FAB 分類の M5b；急性単球性白血病) と診断され AML に対する寛解導入療法が行われた．部分寛解であったが本人の希望により退院し，2003 年 4月からは通院を自己中断していた．2005 年 6 月に右乳房の腫瘤を自覚，乳房腫瘤生検の結果，非ホジキンリンパ腫 (NHL)， びまん性大細胞型 B 細胞リンパ腫 (DLBCL) の病理診断で当科を紹介受診となった．その時に 1999 年の肺生検所見を再検討したところ，腫瘍細胞は CD20陽性，CD3 陰性であり，DLBCL であった可能性が示唆された．その後，NHL に対する化学療法を施行したが，腫瘍のコントロールが不能となり 2007 年 9 月死亡した．NHL と白血病の合併例は今までに …
