Intractable Ingrown Toenails Treated with Bipedicle Nailbed-Periosteal Flap Method : Analysis of 6 Cases

島根大学医学部附属病院皮膚科にて 2016 年から 2020 年にかけて骨棘形成を伴う巻き爪 6 例に対して双茎爪床骨膜弁法を施行し，長期間の経過フォローを行ったので報告する。男性 1 例，女性 5 例，年齢中央値 70.5 歳，6 例とも周辺皮膚軟部組織の炎症を伴って湾曲の強い巻き爪を形成し，CT にて末節骨に骨棘所見が確認された。部分抜爪後の再発が強く懸念され，骨棘切削の必要性から双茎爪床骨膜弁法を施行した。術後フォローは 6±5 年で行い，いずれも経過良好で巻き爪再燃はみられていない。

76 歳，男性。頭部血管肉腫に対しパクリタキセルによる維持化学療法施行中に右上顎第 1 大臼歯が自然脱落した。同部位歯肉に著明な肉芽様腫瘤を形成しており，生検で血管肉腫の転移と診断した。転移巣切除と，術後放射線療法を施行し，現在口腔内に再発所見は認めない。血管肉腫の歯肉転移は稀であるが，未治療の場合は口腔内の易出血性や摂食嚥下障害が危惧される。今回，積極的に治療介入することで良好な局所コントロールを維持することができた。

Skin involvement in adult T－cell leukemia/lymphoma presenting as granuloma annulare－like eruptions

75歳，女性，島根県出身。初診1ヵ月前に，前腕に瘙痒を伴う紅斑が出現し，徐々に多発した。近医でステロイド外用治療をされたが軽快せず，当科を紹介受診した。初診時，躯幹四肢に辺縁堤防状に隆起する環状の浸潤性の紅斑が散在し，環状肉芽腫様であった。皮膚病理組織では，紅斑部では一部表皮内にポートリエの微小膿瘍を認め，真皮に結節状にリンパ球様単核球と組織球を認めた。浸潤している細胞はCD3，CD4，CD25，CCR4陽性であり，CD7は発現低下していた。中央の退色部では，変性した膠原線維の周囲に組織球を認めた。血液検査では白血球11180/μL，異常リンパ球11.3％と上昇を認め，HTLV－1陽性であり成人T細胞白血病リンパ腫と診断した。初診1ヵ月後に異常リンパ球が急増し，CHOP療法2コース，モガムリズマブ併用CHOP療法4コース，モガムリズマブ単体投与を4コース施行しCRに至った。
