透析中におこる血圧低下や下肢筋の硬直対策として, 今日まで, さまざまな工夫がなされてきたが, 頻度は減少していないといわれている. われわれは, 透析前半の急速な溶質除去を抑制することで, 透析中の血圧低下や下肢筋の硬直を予防できるのではないかと考え透析条件の見直しを行った. 結果, 透析中の処置回数の激減と生命予後の指標が上昇したので報告する. 【症例】81歳女性, 透析歴21か月で血圧低下や下肢筋の硬直を頻回に起こす症例である. 【方法】溶質除去を抑えるため, 透析条件はQD 200mL/min, QBは透析開始から漸増する設定 (低効率透析) とし, 血圧低下と下肢筋の硬直の発生頻度と生命予後の指標である, 血清アルブミン, %CGR, BMIについて検討した. 【結果】血圧低下に付随する処置回数は通常透析時, 下肢筋の硬直17回, 除水量の減量19回であったが, 低効率透析に変更後はそれぞれ1回と激減した. 生命予後の指標は, 血清アルブミン値, %CGR, BMIすべて上昇した.

Three cases of the secondary lower-extremity lymphedema treated with intensive drainage by complex decongestive physiotherapy during hospitalization for one week

【緒言】婦人科がん術後に生じた続発性下肢リンパ浮腫急性増悪3例を対象とし, 1週間の入院で2段階方式複合的理学療法の集中排液治療を行った. 【症例】3症例はいずれも子宮体がん術後の女性で, 国際リンパ学会重症度分類stage－II早期であった. 退院時の患肢周径は平均92～96％まで改善した. 症例1 (35歳)は退院後のセルフケアが一時不良となり, 下肢周径の増悪を認めたが, 再教育下でリンパ浮腫ケア外来に通院中で, 浮腫は軽減した. 症例2 (63歳)は退院後もセルフケアが良好で, 下肢周径は治療前の83％まで改善し, 治療前の抑うつ状態も軽快した. 症例3 (70歳)もセルフケア良好で浮腫の再増悪を認めずに経過したが, 治療後1年2カ月でがん再発により死亡した. 【考察】下肢リンパ浮腫に対する1週間の短期入院集中治療は有効であった. しかし, 退院後の維持治療期にセルフケアが不良となって再増悪した症例を認め, 長期的ケアの継続が課題となった.
