Study of Tumor-Associated Carbohydrate Antigens in Colonic Cancers. Immunohistochemical Analysis by Monoclonal Antibodies.

異なる糖鎖を認識する複数のモノクローナル抗体(MoABと略)を用い,大腸正常粘膜,大腸腺腫,絨毛腺腫,早期癌,進行癌,および転移性肝癌でこれらの糖鎖の多様性を免疫組織学的染色法により検討した。各MoABの認識する糖鎖は癌組織の進行度により異なった発現形態を示した.MoAB-SH 1の局在は正常粘膜と大腸腺腫では認められず,絨毛腺腫で強い異型性を示す組織に僅かに局在が認められるが早期癌では癌細胞に有意(27%)に局在を認めた.癌の進展に伴う進行癌(84%)および転移性肝癌(78%)で染色率の上昇が認められた.MoAB-AH 4, MoAB-AH 6でも陽性率はMoAB-SH 1と同様に癌の進行に伴って上昇の傾向を示した.MoAB-FH 6は早期癌では比較的高い陽性率(53%)を認めるが癌の進行により低下の傾向(進行癌=33%,転移性肝癌=33%)を示した.MoAB-TKH 2は進行癌,早期癌とも陽性率は低く2者間に差はなかった.進行癌と転移性肝癌は同一癌組織内に複数のMoABの染色性の混在が認められ,相補分布性(mosaicism)が認められた.これらの結果は癌発生初期のいわゆるcarcinoma in situの状態の糖鎖構造が癌の増殖,進展に伴い,より複雑な糖鎖構造を形成するようになり,癌の多相性(heterogeneity)を,獲得していくものと推察された.
