症例は74歳の男性で，傍大動脈リンパ節腫大と肝転移を有し，胃・脾臓・左腎に浸潤する切除不能膵尾部癌に対して，gemcitabine+nab-paclitaxel療法（以下，GnP療法と略記）を開始した．3カ月後に肝転移・傍大動脈リンパ節転移は消失したため，10カ月後にconversion surgeryの方針とした．術前の分腎機能評価で左腎摘後の予測腎機能は十分と判断し，膵体尾部切除，胃部分切除，脾臓摘出，左腎臓摘出術を施行した．術中造影エコーでも肝転移は完全に消失しており，肝切除は行わなかった．退院後，半年間GnP療法を継続し，術後2年時点で無再発生存中である．傍大動脈リンパ節転移・肝転移の両方が存在した点，術前に腎機能評価を行った上で左腎を合併切除し，術後もGnP療法を継続できた点は既存の報告に類を見ない点であり，ここに報告する．
