A Case of Treated Side Hypoperfusion and Reversible Neurological Deficits Following Carotid Artery Stenting

75歳男性．左ICA狭窄による内科治療抵抗性の脳梗塞に対し，頚動脈ステント留置術（carotid artery stenting: CAS）を施行した．Distal protection deviceにFilterWire EZ（Boston Scientific, Natick, MA, USA）を使用し術中にfibrous capの断裂を認め，回収したフィルター内にdebrisを確認した．術後12時間で右上下肢運動麻痺，意識障害が出現した．広範囲梗塞や脳主幹動脈閉塞は認めなかった一方，123I-IMP-single photon emission computed tomographyで術側の取り込み低下とtranscranial color-coded flow imagingで術側のMCA血流低下を認めた．経時的に術側大脳半球の脳血流は改善しそれに伴い神経症状も改善した．プラーク由来成分の飛散による微小循環障害に起因する低灌流が鑑別として示唆された．

Factors Contributing to Residents’ Interest in Stroke Care: A Questionnaire Survey

【背景および目的】脳卒中診療に関心を持つ若手医師の育成は重要な課題であり，脳卒中診療に興味に与える因子を明らかにすることを目的とした．【方法】対象は当院で研修経験がある卒後1–7年目までの医師とし，質問紙調査を行った．主要アウトカムは当科での研修前後における脳卒中への興味の変化とし，探索的因子分析後に得られた因子を元に多変量解析を行った．【結果】58/85名（68.2%）から回答が得られた．多変量解析では因子（救急外来での診療，個別医師によるマンツーマンの講義，スクラブイン，脳血管造影検査での動脈穿刺などの手技の参加）のみが独立して脳卒中診療への興味増加に関与していた（調節オッズ比7.6，95%信頼区間1.39–57.1，p=0.018）．【結論】実践的な経験の提供と，達成感や自己効力感の獲得を両輪とすることが，脳卒中診療への興味や志望の向上につながる可能性が示唆された．
