〔目的〕周産期脳障害において早期MRIから脳障害特性評価と予後予測の可能性を調査する.〔対象と方法〕療育施設利用者のNICU退院時頭部MRIより在胎週数別に異常部位の視覚的評価と部位別の径計測,また白質異常部位と臨床所見と対比した.〔結果〕すべての群で大脳白質に異常所見が多く,白質部位別では在胎22~26週群では全体,27~32週群では中間部と脳室周囲,33~36週群では中間部,37~40週群では皮質下に異常が多くみられた.白質の皮質下異常では自閉スペクトラム症が有意に多く四肢麻痺と重度知的障害が多い傾向にあった.脳室周囲異常では軽度知的障害が有意に多く痙性両麻痺が多い傾向にあった.〔結語〕大脳白質異常は在胎週数別で特徴があり,予防的リハビリを実施することは重要である.
