76歳男性。Stage IVの右下葉肺癌につき加療中であった。縦隔リンパ節転移による食道狭窄が出現し通過障害改善目的に内科で食道ステント留置を施行した。留置後より腹痛を訴えたため胸腹部造影CTを撮影したところ，食道ステントが腹腔内に穿破しており同日緊急手術を施行した。食道ステントは腹部食道で腹腔内へと穿破しており小網により被覆されていた。ステントを抜去することは困難であったため穿孔部位からさらに胃壁の小弯側を切り足し，穿破しているステントを直接胃内へと誘導したのち穿孔部位と追加切開した胃壁を縫合閉鎖し小網で被覆した。術後経過は良好であり，術後7日目より経口摂取を開始し問題なく経過。その後リハビリ目的に転院となった。腹部食道の穿孔はまれであり，部位別では胸部食道が70.2％と最も多く頸部食道が15.6％，腹部食道が12.2％とされる。調べうる限り，食道ステントによる，腹部食道の穿孔でステントを抜去することなく手術を施行した症例の報告はなかった。今回われわれは食道ステント留置による医原性の腹部食道の穿孔に対してステントを抜去することなく手術を施行し，良好な結果を得たので若干の文献的考察を含めて報告する。

A Case of Desmoid Tumor Suspected to be Seeding Recurrence of Early Gastric Cancer

デスモイド腫瘍は特異的な画像所見に乏しく，診断に難渋する例も多い．症例は45歳，男性．M領域の25mm大の早期胃癌に対し腹腔鏡補助下幽門側胃切除，D1+リンパ節郭清，R-Y再建を施行した．術後診断は胃癌，M，Ant，adenocarcinoma，por and sig＞tub2，pT1b1，ly2，v0，N2（3/29），M0，R0-pStage II Aであり，術後補助化学療法としてS-1療法を8コース施行した．術後3.5年のCTで挙上空腸盲端近傍に15mm大の腫瘤を認め，腹膜播種を疑ったが遡及的に見ると術後2.5年のCTでも同部位に5mm大の腫瘤が確認できた．緩徐な発育であることから，腹膜播種再発以外の可能性も念頭に置き開腹腫瘤切除，審査腹腔鏡を施行した．腫瘤は大網に被覆されていたが周辺臓器への浸潤は無く切除可能であった．病理組織学的には腹腔内デスモイド腫瘍の診断であった．術後20カ月（2019年8月）無再発生存中である．
