乳児期に診断された有症状の横隔膜弛緩症は縫縮術の適応となり，一般にその長期成績は良好である．症例は19歳，男性．乳児期に左横隔膜弛緩症のため開胸縫縮術を施行されている．柔道練習中に突然の左上腹部痛をきたし来院した．CT検査にて左横隔膜外側より横行結腸の胸腔側への脱出を認めたため横隔膜ヘルニアと診断し，腹腔鏡下横隔膜修復術を実施．横行結腸と大網を腹腔側に還納すると，横隔膜縫縮部に一致しヘルニア門が確認され，創に沿うように脆弱箇所を複数認めた．腹圧上昇による横隔膜瘢痕ヘルニアの嵌頓と判断し，ヘルニア門の縫合閉鎖後に周囲脆弱部を含めメッシュにて修復を実施した．特記すべき合併症なく術後第8病日に退院し，術後1年8カ月の時点で再発を認めていない．乳児期に横隔膜縫縮術を施行された場合，成人期においても腹圧上昇に伴うヘルニア発症を考慮する必要がある．
