症例は70歳代，女性．認知機能低下の精査で，血中アンモニア濃度高値や，頭部MRIでT1強調画像での淡蒼球の高信号域が指摘されたことから肝性脳症が疑われた．造影CTで多発する微小な肝内門脈肝静脈短絡症（PVシャント）が指摘された．肝形態は肝表不整，肝実質のエコー像は粗糙であるものの，肝生検では小葉構造は保たれており線維化は指摘されなかったことから，背景肝は非硬変肝と判断した．血管造影検査や経動脈性門脈造影下CTでも，PVシャントが多数指摘された．ラクツロースやリファキシミンでの内服加療で辻褄の合わない発言や易怒性は改善傾向であり，多発微小PVシャントによる肝性脳症が諸症状の原因であったと判断した．多発微小シャントであることから血管内治療は見送り，内服加療での経過観察を継続している．肝内PVシャントは稀な病態であるが，高齢者における原因不明の意識障害では鑑別に挙げることが肝要と思われた．
