A Case of Synchronous and Solitary Adrenal Metastasis of Rectal Cancer Resected Curatively

症例は65歳男性．貧血を主訴に来院．精査の結果，直腸S状部に2型進行癌を認めた．腹部CT検査で左副腎腫瘤が認められ，MRI検査，FDG-PET検査を行い副腎転移が最も疑われた．腹腔鏡下低位前方切除，左副腎摘出術を予定した．直腸周囲のリンパ節が腫大し，原発巣が大きかったため，根治性の面から直腸切除は開腹下に手術を行った．しかし，副腎は安全に腹腔鏡下に切除できた．大腸癌の同時性孤立性副腎転移は報告が少なく，切除可能であれば切除が考慮される．術前に確定診断はできず，切除には診断的意義がある．同時性孤立性副腎転移に対して同時切除は現時点では最も標準的な治療法と考えられるが，今後治療成績を上げるためにさらなる検討が必要である．
