Clinical Practice and Treatment Satisfaction among Patients with Tinnitus in Japan: A Cross-Sectional Web Survey

本邦の耳鳴診療における患者視点での診療実態 (受診行動, 医療機関での説明実態, 治療内容・満足度および医療機関変更意向) を明らかにする目的で本研究を実施した. 本研究は Web アンケートによる横断研究であり, 18歳以上の慢性耳鳴 (病悩期間３カ月以上) で治療意思のある者を対象とした. 調査項目は性別, 年齢, 耳鳴重症度, 病悩期間, 医療機関受診状況に加え, 耳鳴の説明, 治療内容および満足度とした. 今回, 医療機関に現在または過去に通院歴のある耳鳴患者658例について, 診療実態と満足度 (満足およびやや満足の合計) を明らかにした. 耳鳴治療で最初に耳鼻咽喉科を91.8％が受診していたが, 耳鳴の原因, 発生機序や治療選択肢の説明への満足度は14.7～18.4％であり, 治療は薬物療法が主体であった. ガイドラインで推奨される教育的カウンセリングや認知行動療法等の非薬物療法の実施は10％未満であったが, その満足度は教育的カウンセリング33.9％, 補聴器装用33.3％であった. また, 回答数は限定的だが, TRT (Tinnitus Retraining Therapy) 60.0％, 補聴器・TRT 以外の音響療法100.0％であった. 耳鳴治療の不満として47.0％が効果を感じないとし, 25.1％が説明不足や効果実感不足により自ら医療機関を変更していた. 薬物療法中心の現行診療と症状消失を求める患者ニーズには乖離が認められた. …
