Retrospective analysis of percutaneous peritoneal dialysis catheter placement using the Seldinger technique

経皮的腹膜透析カテーテル留置術は局所麻酔で施行できるカテーテル留置法である. 当院では超音波および透視を用いたSeldinger法による同留置術を20名の患者に施行し, 1名で腹腔穿刺ができず断念したが, 腹部手術歴のある6名を含む19名でカテーテル留置に成功した. 腸管穿刺は起きなかった. SMAP法8名, 一期的導入9名 (待機期間6～16日) で腹膜透析を開始しているが, 液漏れは認めていない. 早期合併症として, カテーテル先端位置異常2名, 腹膜炎1名, 血性排液2名のうち1名はカテーテル閉塞に至ったが, いずれも非観血的に対処可能であり腹膜透析が継続できた. 1名で留置15か月後にカテーテル抜去を行ったが, 局所麻酔のみで容易であった. 本留置術は低侵襲であり, 透析患者の高齢化が進むわが国においても有用な腹膜透析カテーテル留置法である.
