Severe acute pancreatitis induced by marked hypertriglyceridemia during a twin pregnancy

症例は27歳女性．妊娠20週0日に心窩部痛が出現し，前医で膵酵素上昇と高トリグリセリド血症を指摘され，当院に紹介された．CTで重症急性膵炎と診断し，入院治療を開始した．胸水貯留による呼吸不全や播種性血管内凝固を続発したが，厳格な脂質制限と輸液管理により改善が得られ，第18病日に退院した．退院後のMRIで急性壊死性貯留を認めたが，感染なく経過し，妊娠36週6日に帝王切開により分娩した．分娩後，血清トリグリセリド値は著明に改善したが，リポ蛋白分画異常は持続し，遺伝的素因と妊娠の両者が原因で高トリグリセリド血症をきたし，急性膵炎を発症したものと思われた．

Lenvatinib-induced severe interstitial pneumonia in a patient with hepatocellular carcinoma: A case report

症例は69歳男性．慢性閉塞性肺疾患の既往あり．肝動脈化学塞栓療法（TACE）不応の肝細胞癌に対してレンバチニブ12 mg/日を開始した．開始3週間後から易疲労感が出現し，減量や休薬を行いながら投与を継続したところ，開始2カ月後に著明な労作時呼吸困難と低酸素血症を認めた．胸部CT所見から間質性肺炎と診断し，入院治療を開始した．レンバチニブ中止後も呼吸状態はさらに悪化したが，プレドニゾロンの内服と高流量鼻カニュラ酸素療法により徐々に改善が得られた．退院から1年後，間質性肺炎の再燃をきたすことなくプレドニゾロンを中止できた．本症例から，レンバチニブが重篤な間質性肺炎を生じうることが示された．レンバチニブ投与中に易疲労感や労作時呼吸困難を認め，減量や休薬で改善が乏しい場合は本副作用を念頭に置く必要がある．また，レンバチニブによる間質性肺炎に対して副腎皮質ステロイドが有効である可能性が示唆された．
