A CASE OF SUCCESSFULLY RESECTED PRIMARY MALIGNANT LYMPHOMA OF THE LIVER

症例は56歳,男性.黄疸を主訴に当院紹介となった.腹部超音波検査, CT検査にて,肝門部に5cm大の腫瘤と肝内胆管の拡張を認めたため,左右両葉にPTBDを挿入し胆管造影を行った.左右胆管の肝門部での完全閉塞を認めた.肝炎ウイルスマーカーは陰性であった.肝門部胆管癌の診断にて,肝拡大右葉切除,胆道再建,門脈再建術を施行した.病理組織学的には異型の強い大型かちやや中型のリンパ球が肝門部を中心として肝,胆嚢へ浸潤増殖していた.免疫染色にてLCA(+), L26(+), UCHL(-), EMA(-) であり, non-Hodgkin lymphoma(diffuse large B cell type)と診断された.稀な疾患である肝原発悪性リンパ腫の1切除例を経験したので,若干の文献的考察を加えて報告する.
