A Case of Minimally Invasive HER2-Positive Breast Cancer Metastasizing to the Lung after Surgery

乳腺腫瘍の組織学的分類には浸潤癌・非浸潤癌・Paget病のほかに，浸潤径1mm以下の微小浸潤癌が分類されている．微小浸潤癌は非浸潤癌に近い良好な予後が報告されているが，稀に遠隔転移をきたすことがある．本症例は，微小浸潤癌の根治手術後に孤立性肺転移をきたした．また，本症例はHER2陽性であったが，腫瘍径1cm以下のHER2陽性乳癌に対して化学療法とトラスツズマブを使用する十分な根拠はないため，本症例も化学療法を行わず経過観察としていた．術後1年後に孤立性肺病変を確認したため，少数転移の治療として病変を摘出した．病理結果は乳癌の肺転移であった．術後に化学療法を含む抗HER2療法を追加し，5年経過の時点で転移再発の兆候はない．文献上，微小浸潤乳癌の孤立性移転に対して少数転移として局所制御目的に摘出手術が施行された報告例は本症例以外には見当たらないため，この度文献的考察を加えて報告する．

A Case of Breast Cancer Complicated with Silicone Granuloma after Breast Augmentation

症例は79歳，女性．左乳房に腫瘤を自覚し，当院を受診．左乳房に6 cm大の発赤を伴う腫瘤を認めた．過去に両側乳房にシリコン注入による豊胸術を施行していた．超音波検査ではシリコンによる無エコー構造物の間に低エコーで不均一な腫瘤像を認め，同部位に針生検を施行したところ浸潤癌の診断となった．造影MRI検査では左乳房に造影効果を伴う6 cm大の腫瘤を認め，左腋窩リンパ節の軽度腫大を認めた．MRIで左乳房内を占拠する肉芽内に造影部を認めた．明らかな遠隔転移は認めなかった．左乳癌の診断で，左胸筋温存乳房切除術+左センチネルリンパ節生検を施行した．病理所見ではシリコン肉芽腫内に腫瘍細胞の増殖を認め，浸潤性乳管癌と診断された．センチネルリンパ節内にもシリコンの混入が認められたが，転移陰性であった．以上，豊胸術後のシリコン肉芽腫に併発した乳癌の手術症例を経験したので，文献的考察を加え報告する．
