〔目的〕前十字靭帯（ACL）機能温存型の人工膝関節全置換術（TKA）後の歩行と階段昇降能力に影響する術後因子を明らかにすること．〔対象と方法〕対象は，ACL機能温存型のTKA後患者40例とした．移動能力は，術後の歩行能力をTimed Up and Go Test，階段昇降能力をTimed Stair Testにて計測した．術後因子は，膝関節角度と筋力とした．〔結果〕歩行能力に影響する因子は，膝関節伸展筋力（p＜0.01，R2＝0.34）であり，階段昇降能力に影響する因子も膝関節伸展筋力であった（p＜0.01，R2＝0.11）．〔結語〕術後の膝関節伸展筋力は，ACL機能温存型TKA後の歩行および階段昇降能力に影響を及ぼす因子であった．

Examination of Preoperative Factors Predicting Cases of Muscle Weakness 6 Months after Total Knee Arthroplasty

〔目的〕本研究の目的は，人工膝関節全置換術（TKA）後6ヵ月の筋力低下例を予測する術前因子を明らかにすること．〔対象と方法〕対象は，変形性膝関節症の診断で片側のTKAを施行した患者108名とした．筋力は，術前，術後2週，3ヵ月，6ヵ月時に最大等尺性膝伸展トルクを体重で除した値を計測した．術前因子は，基本属性，膝可動域，大腿部筋形態，筋力を抽出した．重回帰分析にて，術後6ヵ月の筋力値に影響する術前因子を解析した．〔結果〕術前の大腿部筋横断面積と筋力が術後6ヵ月の筋力を予測する因子であった（p＜0.01，R2＝0.45）．〔結語〕TKA後6ヵ月時に筋力低下が残存する症例は，術前に大腿部の筋萎縮と筋力低下を認めている可能性が高いことが示唆された．

〔目的〕両十字靭帯温存型の人工膝関節全置換術（TKA）後患者では，歩行の荷重応答期に正常な膝関節伸展筋群での衝撃吸収機能を発揮できているのかを検証すること．〔対象と方法〕対象は，術後1年経過したTKA群8名，同年代高齢者の健常群12名とした．三次元動作解析にて歩行時の矢状面における下肢関節運動学・運動力学データを計測した．〔結果〕歩行の荷重応答期における床反力鉛直成分，膝関節伸展モーメント，遠心性膝関節パワーにおいてTKA群は健常群よりも有意に低値であった．歩行時の膝関節屈曲角度では，TKA群は健常群よりも低い傾向を示した．〔結語〕両十字靭帯を温存してもTKA後患者では，歩行時に膝関節伸展筋群での衝撃吸収機能を十分に発揮できていなかった．
