A Case of Parathyroid Adenoma Presented with Descending Mediastinal Hematoma

症例は69歳，女性．咽頭痛と嚥下困難で近医を受診した．胸部CTで長径約50mmの上縦隔腫瘍を認め，当科を紹介受診した．胸部造影CTでは造影効果を伴う上縦隔腫瘍と造影剤の血管外漏出を認め，縦隔腫瘍穿破と診断し緊急で胸腔鏡下手術を行った．胸腔内を観察すると上縦隔に腫瘍は無く，縦隔胸膜に包まれた血腫を認めた．縦隔血腫は上縦隔から頸部に連なり，縦隔の血腫除去術を行ったが胸腔内に出血点は認めず，出血は頸部から降下していたため，体位変換後に頸部手術を行った．甲状腺右葉下極の背側に血腫と腫大した副甲状腺を認め，副甲状腺腺腫出血と診断して血腫除去と右副甲状腺腺腫摘出術を行った．術後は経過良好で7日目に退院となった．病理診断は副甲状腺腺腫の診断で，縦隔血腫内にも副甲状腺腺腫細胞が混在しており，副甲状腺腺腫出血による降下性縦隔血腫と診断した．副甲状腺腺腫出血による降下性縦隔血腫は稀な疾患であり，文献的考察を加えて報告する．
