15年以上繰り返すイレウスの原因診断に難渋し,梅の種子による食餌性イレウスを契機に診断しえた1例を経験したので報告する.症例は77歳の女性で,腹痛を主訴に近医を受診し精査目的にて当院紹介となった.腹部単純X線では軽度のニボーを認め,腹部CTでは拡張した小腸と腸管内の石灰化異物が認められた.イレウス管を挿入し保存的に経過観察したが,イレウス管の進行が止まったため小腸造影を施行したところ, 2カ所の小腸狭窄が認められ,腸管内には楕円形の陰影欠損像が認められた.小腸狭窄を伴った異物によるイレウスと診断し開腹手術を施行した.回盲弁より約40cm口側の回腸に2カ所の狭窄が認められ同部を切除した.切除腸管を切開したところ,数個の椎茸片と2個の硬い種子様の異物が認められ,後者は石灰化した梅干しの種であることが判明した.小腸狭窄を伴い梅干しの種に起因した食餌性イレウスと診断した.
