Spiritual Care Training Program’s Effects on Physicians’ Confidence, Self-reported Practice, and Attitudes in Caring for Terminally Ill Cancer Patients Who Express Meaninglessness

【目的】スピリチュアルケア（SPC）の実践力の習得を目的とした研修の医師での効果を測る．【方法】自記式質問法により，教育介入前，直後，3カ月後，6カ月後に測定．【結果】医師30名が研修を修了．すべての主要評価項目が有意に改善し，その効果は介入6カ月間持続（すべてP=0.0001）．スピリチュアルペイン（SPP）を訴える患者とのコミュニケーションの自信が高まり（6カ月後の効果量（Effect Size=1.3），SPCの実践の自己評価が高まり（ES=1.2），SPPを訴えられたときの無力感が軽減し（ES=0.8），SPCの経験を肯定的に捉えるようになり（ES=0.8），SPPを訴える患者にすすんで関わりたいと思うようになった（ES=0.4）．96〜100％の医師が，SPCの概念理解と実際にSPCの方法を知ることについて本研修が「とても役に立った」または「役に立った」と評価した．

The Structure of Existential Suffering in Physicians Caring for Terminally Ill Cancer Patients

【目的】がん患者の終末期医療に携わる医師のスピリチュアルペイン（SPP）を明らかにする．【方法】医師の臨床体験レポートを記述現象学と3次元存在論で分析した．【結果】すべてのレポートで医師のSPPが抽出され，時間性，関係性，自律性に分類された．とくに医師の意識の志向性が，がん治療や症状緩和の限界や患者の訴えるSPPに対応できないことに向けられ，それが医師としての無力・無能として現れる自律性のSPPが大多数を占めた．自律性のSPPの体験の意味と本質は，［治療（キュア）の限界に直面している自己が無力として現れる］［患者のSPPに対応できない自己が無力として現れる］［自分を取り巻く外的な環境の問題（過重労働や教育の不備など）が原因で自己の無力が生じる］という三つの構造で示された．またキュアの限界で医師が患者に会いづらくなる，避けるという体験は医師の自律性のSPPへの対処（コーピング）の可能性が示唆された．
