A Case Report of Small Cell Carcinoma of the Esophagus.

食道に発生した小細胞癌の症例を経験したので報告する.患者は62歳の男性で嚥下障害を主訴に近医を受診し,内視鏡検査で胸部下部食道に潰瘍性病変を認めた.術前に2回の生検検査を施行したが,異型細胞を認めるものの確定診断には至らなかった.画像診断より進行食道癌と診断し, CDDP, 5FUの術前化学療法を施行した後に根治手術を施行した.摘出標本では下部食道に6×3cmの腫瘍を認めた.病理組織学的には,顆粒状のクロマチンと核小体を有する多型性の核と少量の胞体を持つ細胞がびまん性に増殖していた.また腫瘍周囲のごく一部に低分化な扁平上皮癌の像も認められた.びまん性に増殖していた腫瘍細胞は免疫染色にてNSEとSynaptophysinが陽性で,電子顕微鏡では内分泌顆粒を認めた.食道癌取扱い規約における小細胞癌の所見であった.
