超高齢社会を迎え，慢性膵炎診療においても高齢者医療の需要はますます高まってきている．最新の全国調査の解析から，本邦の慢性膵炎患者の17.6%が75歳以上の高齢者であり，高齢者慢性膵炎の成因はアルコール性が少なく，特発性の頻度が多いことが明らかとなった．高齢者慢性膵炎は無痛性が多く，膵外分泌機能障害の合併に注意する必要がある．高齢者ではフレイル，サルコペニアやオステオパシーなど合併症の観点から，非代償期の症例に対しては十分な膵消化酵素補充療法と脂肪を制限しない食事指導が重要となる．また，高齢者慢性膵炎では，膵癌を含めた悪性腫瘍の合併にも留意する必要がある．これらの高齢者慢性膵炎の特徴に加え，加齢に伴う生理的な変化，複数の併存疾患の存在，薬物療法による有害事象，社会的要素といった高齢者特有の問題を考慮しながら，個々の患者に合った丁寧な診療が求められる．
