A Case Report on Laparoscopic Duodenal Segmentectomy for a GIST of the Horizontal Duodenal Limb

症例は73歳，男性．当院消化器内科で分岐型IPMNフォロー中にEUSにて十二指腸水平脚に22mm大の壁外腫瘤があり，EUS-FNAを施行してGISTの診断となり，手術目的に当科に紹介となった．造影CTでも十二指腸水平脚に多血性腫瘍を認め，腹腔鏡下にて十二指腸分節切除を施行した．手術は小腸間膜背側から後腹膜アプローチを用いて十二指腸の授動を行い，腫瘍の口側と肛門側の腸管を切離して標本摘出後，体腔内にて機能的端々吻合を施行した．術後経過は良好で，術後7日目に退院となった．病理組織学的にlow risk GISTと診断され，破裂はなく完全切除できていた．十二指腸GISTは比較的稀な疾患であり，その解剖学的特異性から腫瘍の局在，大きさ，性状により様々な術式が行われている．十二指腸水平脚GISTに対する腹腔鏡下分節切除において，後腹膜アプローチよる授動と機能的端々吻合による再建は有用な選択肢と考えられる．
