Laparoscopic Surgery for Rectal Cancer with the Inferior Mesenteric Artery Arising from the Superior Mesenteric Artery

症例は66歳の女性で，下血を主訴に前医受診し，精査の結果直腸癌と診断され，手術目的で当院紹介となった．術前の腹部造影CTで，下腸間膜動脈（inferior mesenteric artery；以下，IMAと略記）は上腸間膜動脈（superior mesenteric artery；以下，SMAと略記）より分岐し，下腸間膜静脈沿いに下行していた．直腸癌RaRb，cT2N0M0，cStage Iの術前診断で，腹腔鏡下低位前方切除術，一時的回腸人工肛門造設術の方針とした．術中所見では術前の画像診断同様，IMAの根部は欠損しており，同部には腰内臓神経の結腸枝のみを認めた．IMAは下腸間膜静脈と伴走し，頭側へ走行していた．郭清の上縁は腰内臓神経の結腸枝の部位とし，その足側から分枝する左結腸動脈を温存し，その末梢でIMAを切離した．術後は乳糜漏を認めたが，保存的治療で改善し，術後10日目に退院となった．SMAよりIMAが分岐する形態は非常にまれであり，ここに報告する．
