症例は72歳，男性．左鼠径部膨隆を主訴に前医を受診し，左鼠径ヘルニアの診断で当科紹介となった．左鼠径部に立位でピンポン球大の膨隆を認め，臥位で軽快したが，陰嚢の腫大は残存したため，腹部造影CT検査を施行したところ，膵体尾部癌，傍大動脈リンパ節転移を認めた．腫瘍の浸潤により左腎静脈は閉塞し，途絶した左腎静脈から陰嚢にかけて左精索静脈瘤の発達を認め，陰嚢腫大の原因と考えられた．
