症例は66歳，男性．慢性糸球体腎炎による慢性腎不全のため，24年前に透析導入となっている．高血圧および高尿酸血症を合併しており，また，両側手根管症候群に対して手術の既往を有している．さらに，シナカルセト投与前よりアルコールによる中等度の肝機能障害を認めていた．経過観察中にintact-PTHが371 pg/dLと上昇し，活性型ビタミンD3製剤の経口投与を行っているにもかかわらず，intact-PTHが高値で推移していたため，シナカルセト25 mg/日の投与を開始した．投与開始4日目にはAST 1,709 IU/L，ALT 561 IU/L，LDH 1,602 IU/Lと逸脱酵素が著しく高値となった．腹部CTでは，肝腫大および胆石を認めたが，それ以外に肝内胆管あるいは総胆管の拡張や肝実質の器質的異常は認められなかった．アルコール性肝障害の急性増悪も否定はできなかったが，新たに投与を開始したシナカルセトによる薬物性肝障害が最も疑われたため，投与を直ちに中止した．中止1週間後の逸脱酵素はAST 49 IU/L，ALT 61 IU/L，LDH 172 IU/Lと著しく低下した．DDW-J 2004薬物性肝障害ワークショップの診断基準のスコアによると，初回投与から発症までの期間，好酸球増多，投与中止後のデータなどより，肝細胞障害型で，スコアは7点と，5点以上となり，シナカルセトによる薬物性肝障害と診断した．中止2週目には肝逸脱酵素は正常に復していた．シナカルセトによる肝障害の報告はまれで，肝逸脱酵素の上昇も1％未満とされている．自験例のように，投与開始近接期に急激に肝逸脱酵素が上昇し，投与中止および肝庇護剤の投与のみで，正常に復した報告例はなく，きわめてまれな症例を経験したので報告する．
