Physical Therapy for Compound Knee Ligament Injury After Simultaneous Reconstruction of Anterior and Posterior Cruciate Ligaments

膝複合靭帯損傷により，前十字靱帯 (anterior cruciate ligament: ACL) 及び後十字靱帯 (posterior cruciate ligament: PCL) 同時再建術を施行された1症例に対し理学療法を行なった。 術後プログラムは，1996年の遠山の報告を基にし，さらに当院のACL及びPCL単独損傷後における各術後プログラムを組み合わせ作成し，理学療法 (physical therapy: PT) を行なった。〔結果〕術後4か月で膝可動域 (range of motion: ROM) は正常となった。術後16か月で，膝不安定性は認めず，筋力はピークトルク・180°/sec・患健比で大腿四頭筋 (quadriceps femoris: Quad) は87%，ハムストリングス (hamstrings: Ham) は103%，膝運動能力は下肢機能的運動能力テスト (functional ability test: FAT) で4種目全てが良好となった。 ACL及びPCL同時再建術後PTのポイントは，以下の3つである。第1に，再建靭帯 (特にPCL) の最低限の成熟が期待できる時期まで，ACL及びPCLのストレスを最小限に抑えるため，Quad・Hamの同時収縮を中心とした筋力強化を膝軽度屈曲位で行なうこと，また，Hamの単独収縮を避けることである。第2に，膝拘縮により授動術を要する可能性が高いため，主治医と膝ROMの回復経過について密に連絡をとることである。第3に，膝運動能力向上のために，協調性トレーニングを十分に行なうことである。
