A CASE OF GASTROINTESTINAL STROMAL TUMOR OF THE RECTUM WITH BONE METASTASIS

骨転移をきたした直腸原発gastrointestinal stromal tumorの1例を経験したので報告する.症例は77歳,女性.下血,貧血を主訴に来院し,直腸診,骨盤部CT検査および注腸検査にて肛門縁より2cm, 直腸Rb部に径6cmの腫瘍を認めた.直腸間葉系腫瘍と診断し,腹会陰式直腸切断術を施行した.病理組織学的検査にて直腸平滑筋肉腫と診断され経過観察してきたが,術後14カ月目にTh10, L3に異時性骨転移を認め,術後20カ月目に永眠された.後日改めて施行された特殊免疫染色の結果CD34, c-kit陽性, SMA, S-100陰性を認め,直腸原発GISTと診断した.直腸原発GISTの骨転移症例は非常に稀であり,本邦直腸GIST報告例の検討も含めて報告する.
